About the firm

What is Avina, in one paragraph?

Avina Wellness is a private wellness firm. Four clinical practices sit beneath one umbrella, coordinated for each principal by a single Chief of Staff. Medical detox in residence. Companions and case managers. Therapeutic IV. Telehealth therapy and family work. Discretion is the product. Everything else is delivery.

Who is Avina for?

Principals at significant wealth and the family offices who serve them. Most enquiries come from family-office principals, general counsel, trustees, and senior advisors who need a peer firm to hold the medical and behavioural complexity their office cannot.

Where does Avina operate?

California, New York, and Florida in Year One, with London and additional jurisdictions added as licensure permits. Cases outside these jurisdictions are considered in coordination with local clinicians.

How big is the book?

Twelve concurrent principals at steady state. Not a hundred. Not a thousand. Twelve, held by a Chief of Staff each, by a clinical bench each, by an office that knows them by name.

Avina Home Detox

How quickly can a detox begin?

We have begun detoxes within twenty-four hours of first enquiry where the case warranted it. Most begin within three to seven days, after the Medical Director’s case review, the engagement letter, and the nursing schedule are in place.

Is in-residence detox safe?

For carefully screened candidates, yes. The Medical Director declines cases where the safest setting is a hospital. Screening covers severity of dependence, prior withdrawal history, cardiac and seizure history, comorbidities, residence environment, and the support around the principal.

Which substances do you treat?

Alcohol, benzodiazepine, opioid, polysubstance, and stimulant withdrawal under indication-specific protocols governed by the Medical Director.

What if there is an emergency?

The protocol is to call 9-1-1 first and notify the physician of record second. Nurses are trained to escalate within minutes of any change in condition. The Medical Director and the Director of Nursing are reachable around the clock for the duration of the engagement.

Chief of Staff

What does the Chief of Staff do?

Holds the operational spine of the principal’s life: medical, legal, recovery, household, relational, professional. The Chief of Staff coordinates the four Avina practices, plus a curated network outside the firm, so the principal hears one voice and the family hears one voice.

Is the Chief of Staff a case manager?

No. The Chief of Staff is the most senior operator the firm fields, with a decade or more of experience holding principals at this altitude. Case managers and companions sit beneath her and report up.

How long does an engagement run?

The contract is open-ended. The engagement moves through four states (Intake, Active, Managed, Steady). Steady is the state most principals occupy for the longest. The relationship endures.

Pricing and engagement

What does it cost?

Avina does not publish a list. Engagements are priced on review of the case: the practices commissioned, the geography, the staffing intensity, the engagement state. Chief of Staff retainers sit in the range of leading concierge wellness firms; Home Detox is priced by the case. The basis is explained on first conversation.

Do you accept insurance?

Avina is a private-pay firm. Where reimbursement is appropriate, the Avina Wellness LLC entity issues documentation that families and family offices may submit to their carriers. We do not take insurance assignments.

How do I begin an engagement?

Submit the enquiry form. Read personally by Kelly Mortazavi or her senior operator. Reply within forty-eight hours. The first conversation is an hour by phone. If we proceed, an engagement letter is issued. We do not enrol every case that calls.

Discretion and data

Will the family know what the principal tells you?

Only what the principal authorises in writing. The engagement letter names the family contacts who receive briefings, the rhythm of the briefings, and the boundary of clinical confidentiality between principal and family. The default is privacy.

How are records held?

HIPAA-eligible encrypted infrastructure under the Avina Wellness LLC entity. Granular access. NDAs covering every clinician and vendor on the case. Records are retained for the legally required period and then permanently deleted under a verified right-to-forget cascade.

Will Avina ever name a client?

No. Not in press, not in marketing, not in conversations with prospective clients. The firm’s reputation is the residue of that behaviour over time.
